Patient Information

Introduction

Oral and maxillofacial surgery requires additional years of hospital-based surgical and anesthesia training after graduation from dental school. After completing dental school, Dr. Lebedovych completed an additional 7 years of advanced training.  As an oral and maxillofacial surgeon, Dr. Lebedovych manages a wide variety of problems relating to the mouth, teeth, and facial regions.  Foothills Oral and Facial Surgery treats a full scope of oral and maxillofacial problems with expertise ranging from placing dental implants, oral and dermatologic pathology, wisdom tooth removal and impacted teeth. This also includes techniques designed to rebuild bone structure with minimal surgical intervention and optimal patient comfort. We offer intravenous sedation or outpatient general anesthesia in a state-of-the-art office setting. 

Our Staff

Our surgical staff is well trained and includes certified surgical technologists. In addition, our administrative staff is experienced and well-versed in health and dental insurance policies. Our entire staff is CPR-certified. We have staff members who are trained and certified in advanced cardiac life support, anesthesia assistants, and patient care. In addition, our entire staff is Basic Cardiac Life Support certified. In addition, Dr Lebedovych as completed an anesthesia fellowship with the Department of Anesthesia and Critical Care Medicine at Johns Hopkins.  In all, he has completed 18 months of anesthesia training.

First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any X-rays, if applicable,.
  • A list of medications you are presently taking.
  • If you have medical and/or dental insurance, bring your cards along with information on the insured party. This will save time and allow us to help you process any claims.


IMPORTANT:
All patients under the age of 18 must be accompanied by a parent or Legal guardian.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.

Financial Policy

We accept cash, check, Visa, MasterCard, care credit or Discover. Payment is due in full prior to services being rendered for patients with no insurance. For patients with insurance coverage: Your consultation fee and fees for any x-rays taken are due in full on that day of service; those charges will be filed with your insurance which may trigger a reimbursement for the cost of consult/x-ray charges. Your surgery fees will also be filed in full with your insurance.  We require an upfront deposit of $200 to reserve your surgical appointment.  This deposit will be applied to your out-of-pocket expenses. Please note that after we receive a response from your insurance company, you will receive a statement from us.   This statement will reflect if you owe any additional money or if our office should owe you a refund. 

**We make every effort to calculate the most accurate out-of-pocket expenses.  We generate these figures based on what your insurance company provides us. We routinely secure reference numbers and names of the agents that provide us the insurance breakdowns.  Unfortunately, whether intentionally or unintentionally, insurance companies  can  provide incorrect information which can greatly affect any balance that the patient may owe.**

As a courtesy to our patients, we will gladly file your insurance for you. We are equipped to file your claims electronically, which accelerates the payment of your claim. The total charge for your care is your responsibility, whether your insurance company pays or not. We cannot bill your insurance company unless you provide us with the correct information and allow us to copy your card. Your insurance is a contract between you and your insurance company; we are not a party to that contract. Our office is not a participating provider with Medicare or Medicaid; therefore, we will not file any claims to them.

Although we do not offer any in house financing options, we do offer care credit with interest free financing for 6 months.  If you ever have any questions regarding our financial policy or your account, please contact us. Many times, a simple phone call will clear up any misunderstandings.

Scheduling

Foothills Oral & Facial Surgery is open Monday-Thursday from 8 a.m. until 5 p.m. and on Friday from 7:30 a.m. to 12:00 p.m. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.

We try our best to stay on schedule to minimize your waiting. Due to the fact that Foothills Oral & Facial Surgery provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding.
